20th Annual Fall Forum for Local Education Foundations

Click the link below to download the registration form for the 2008 Fall Forum for Local Education Foundations. When you register for the Fall Forum, you will receive admission for all sessions, continental breakfast, lunch and handouts – all for just $50 per person, if you register by Oct. 10. Late registration is $60 per person. Registration for Clark’s opening address only is $10 or $5 each for groups of five or more. Tickets will not be mailed. Reservations will be held at the door.

Complete the registration form, making a separate copy for each person who plans to attend. Include payment by check or school district purchase order. This form and payment must arrive at the Foundation office by Friday, Oct. 10, to avoid a late registration fee.
